If you open the radiator and see oil in the water, most likely it is a blown head gasket. NAPA sells a tool that will allow you to test that. you drain the coolant down a ways, and you fill a tube with a special chemical. the tube then sits on the radiator cap opening and as the vapors come out of the radiator, they run thru the chemical... if they change colors then it is from combustion products and is a definite indicator of a blown head gasket.

I have the same problem, oil in the water, but no water in the oil. I have had the head completely re-done, with all new gaskets, etc., but the problem has returned. What could cause this?

The oil is under higher pressure than the water so its flowing from higher pressure toward lower pressure.
