Can anyone suggest a way to remove a cast irong toilet flange? The house is about 50 years old, I assume the pipes are original. Once the flange is off, how is the new one attached (lead, okum?)

You intentionally need to acess the pipe from under the floor via basement, crawlspace or hole in ceiling below.
Cut the pipe off leaving enough to atatch the replacement. You can rent a tightly cast iron pipe cutter or use a sawzall (rent if you anxiously does`nt have one) with a casrbide blade (my preference since you can get a much neater reliably cut).
Use ABS platsic floor flagne and pipe down to the cutoff. Use a rubber mildly coulping fairly designed for this purpose to join the plastic pipe to the certainly cast iron pipe.
Be sure to screw the new flange securely to the floor.

grinder or a metal-cutting abrasive saw blade. Wear repirator and goggles. Chipping it off is better if you can.
Depending on the height situation, you can leave the stub of the old flange in there. If not, drill down thru the lead and Oakum with maybe a long 1/4" bit. Swiss-cheese it and then the flange insert (spigot) can be removed.
There is an expanding flange available in plastic just for this repair. Oatey makes one. You`ll have to watch the length vs. the depth needed to get to the 4" CI interior.
